What are the responsibilities and job description for the WERA Program Case Manager position at WDEOC?
The WERA Program Case Manager is responsible for case management services in the WDEOC/WERA service area to include conducting needs assessments and formulating individual development plans; identifying and making referrals to other resources; serving as a clearinghouse for referrals to subsidized housing, serving as an advocate for clients; provision of appropriate counseling services and follow-up activities; assistance in identifying and securing permanent housing; coordinating services with agency programs and other organizations; developing and maintaining reports and files, and maintaining high level of data quality

This position requires a bachelor’s degree in Human Services or related field or at least four years experience in social work, counseling, or related field. Previous experience working with individuals with mental illness and programs in a case management capacity (minimum one year) required. The starting salary is $41,142 per year based on experience.
