Case Manager (LPN)
Case Manager (LPN) coordinates the overall care plan for patients under the direct supervision of a Registered Nurse (RN) or physician. Evaluates the needs of the patient, the resources available, and recommends the plan for the best outcome. Being a Case Manager (LPN) collaborates with physicians and other medical staff members to facilitate the progression of care. Monitors the patient's progress and acts as a liaison between the patient, medical staff, and insurers. Additionally, Case Manager (LPN) develops a discharge plan that provides the best available resources to meet ongoing patient needs. Identifies patient care issues and makes recommendations. Requires a high school diploma or equivalent. Requires graduation from approved LPN educational program. May require a state license. Typically reports to a Registered Nurse (RN) or supervisor. Case Manager (LPN)'s years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement.
