Case Manager - Physician Practice coordinates the overall care plan for assigned patients in a physician practice setting. Evaluates the needs of the patient, the resources available, and recommends the plan for the best outcome. Being a Case Manager - Physician Practice collaborates with physicians and other medical staff members to facilitate the progression of care. Monitors the patient's progress and acts as a liaison between the patient, medical staff, and insurers.
