Job Description Job Description Responsibilities: Provide face to face peer services to people with substance use disorder and other behavioral health challenges and linking them to needed community resources. Complete and maintain accurate records and reports regarding the patients' histories and progress, services provided, and other required information. Intervene as advocate for patients or patients to resolve emergency problems in crisis situations. Plan and implement follow-up and aftercare programs for patients to be discharged from treatment programs. Coordinate activities with primary counselor, courts, probation officers, community services and other post-treatment agencies.
