Position Summary
Reporting to Nursing Leadership, the Registered Practical Nurse will provide excellent nursing care to residents at the Lakeridge Gardens 320 bed long term care home being built on the Ajax-Pickering hospital site.
This new home is part of the government's plan to improve long-term care delivery and capacity in Ontario and in the Durham Region. This state-of-the-art long-term care home sets a new bar for seniors' care, with dining and outdoor space on every floor, a large courtyard, onsite diabetes care and other important innovative features.
Just as Lakeridge Health is a beacon in the Durham community, the new long-term care home will be integral to managing and delivering on the needs and expectations of seniors in the Durham community and Ontario.
Duties And Responsibilites
- Assessing, planning, implementing and evaluating to provide high quality nursing care to residents;
- Assisting in the development and maintenance of an appropriate plan of care for each resident Ensuring resident care plans are initiated in collaboration with other members of the multidisciplinary team
- Communicating with relatives regarding the care and condition of respective residents as required
- Participating in discharge planning, consultation, and/or transfer of residents
- Providing provide direct resident care and participate in resident teaching as required
- Encouraging active participation by residents in all therapy programs and recreational activities
- Providing first aid to staff when necessary and completing Employee Injury Reports
- Procuring and administer medications including injections and narcotics
- Participating in the orientation, on-the-job training and in-servicing of nursing staff as required
Qualifications
- Certificate of Registration in good standing with the College of Nurses of Ontario as a Registered Practical Nurse
- Gentle Persuasive Approaches (GPA) in Dementia Care, DementiAbility training, and PIECES Learning and Development program successfully completed;
- Nonviolent Crisis Intervention Training successfully completed within first three months of employment;
- Related experience in a geriatric or Long Term Care environment is preferred;
- Demonstrated commitment to maintain Reflective Practice Standards as outlined by the CNO;
- Demonstrated knowledge of and compliance with safe work practices
- Must possess strong written and oral communication, critical thinking, care coordination, time management and customer focus skills;
- Demonstrated empathy, concern and interest in providing quality care and service for residents
- Physical ability to perform the essential duties of the job

- Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-services in this area
Conditions Of Employment
The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and C.U.P.E Local 6364.
