POSITION SUMMARY:

Under the supervision of the Director of Nursing and/or the Administrator, this person will be responsible for:

· Coordinating, planning, organizing, and overseeing the RAI process following home policies, MLTC regulations, College of Nurses of Ontario, and interRAI requirements.

· Will assume other responsibilities as delegated by the Director of Nursing for staff development, orientation, infection control, best practice policy and procedure development, and quality improvement activities.

KEY RESPONSIBILITIES:

· Participate in the planning and organizing of the Home RAI process change

· Monitor and provide on-site support to all RAI users in the Home

· Communicate regularly with the RAI users and management team

· Provide RAI-MDS 2.0 education and computer application training to all RAI users

· Ensure a resident assessment schedule is developed, completed, current, and accessible to all RAI users

· Assist RAI users to correctly code assessments and provide support for validation of coding

· Work with RAI users to develop processes that ensure data accuracy, efficient data collection process, assessment completion, and data utilization

· Ensure MDS assessments, resident assessment protocols, and care planning are completed promptly

· Provide on-site computer application support to all RAI users in the home

· Oversee the development and coordination of the data submission process

· Attend the Data Submission Web-Ex training by CIHI and the Project

· Review, prepare, and submit RAI data quarterly to the Canadian Institute for Health Information (CIHI)

· Generate and review RAI-MDS 2.0 Output Reports with the leadership team, front-line staff, and PSWs and facilitate their use in clinical and operational decision-making

· Complete and submit all required RAI implementation assignments timely to the Project Support Centre

· Provide ongoing leadership for education, data accuracy, data submission, and computer applications of RAI

QUALIFICATIONS:

· Current certificate of competence as a Registered Practical Nurse with the College of Nurses of Ontario.

· Comprehensive knowledge of the assessment and care planning processes.

· Proficient computer skills and well-organized with excellent time management skills.

· Demonstrated success in leading new initiatives

· Knowledge and experience with adult education strategies/facilitation

· Clear aptitude for detail-oriented work

· The ability to work with and relate to interdisciplinary team members and other internal stakeholders

· Strong problem-solving abilities

  • Well-developed oral and written communication skills

· Long Term Care experience would be an asset.

- Must provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one

JOB TYPE: Temporary Full-Time
