Position Overview
The Program Leader, Home Maintenance & Repair (HMR) is accountable for managing the delivery and efficient operation of the HMR referral program. The program refers screened providers to clients in the community who have requested one of HMR’s services. Accountabilities include HMR provider recruitment and screening and relationship management; employee supervision; client work and satisfaction; accurate and timely administration; health and safety; community relations and professional development.
As a referral model program, the Program Leader is responsible to provide key supports to providers who are screened by Links2Care but are independent workers and are not employees of Links2Care.
Job Type: Full Time Position, 8:30 to 4:00 PM (M-F)
Location: Oakville, ON (On-Site) and Community (Oakville, Burlington, Halton Hills, Milton, Mississauga and Etobicoke)
Closing Date: March 15, 2024
Target Compensation Range: $55,000 to $65,000/ Year
Role and Responsibilities
· The Program Leader, Home Maintenance and Repair (HMR) is responsible for the overall program operations and KPIs, as well as employee supervision.
· Recruit, interview, select and onboard qualified providers to carry out program activity safely and efficiently.
· Maintain confidentiality of all pertinent agency documentation/information.
· Screens new referrals of new clients to determine client goals and service eligibility.
· Coordinates services for clients, continually monitoring and adjusting service to address clients’ changing goals.
· Complete all administrative tasks tied to supervision inclusive of payroll, yearly performance review, mileage, expenses and other relevant forms and requests.
· Reports on and monitor program and project activities, outputs and progress.
· Collect and maintain client and service provider information based on program needs and employer requirements in accordance with the Links2Care policy and relevant legislation.
· Maintain positive relationships with networks, professional organizations, and other related service providers.
· Represent the mission, values, and philosophy of the agency in the community, including networks or meetings as requested.
· Conduct outreach and promote Links2Care programs in the community to build awareness and increase client numbers.
