GENERAL PURPOSE OF JOB: The staff nurse (RN) is a professional Registered Nurse who provides direct nursing care to assigned patient population. The staff nurse, utilizing the nursing process, plans and delivers effective patient care within the Texas scope of practice and within the framework of Driscoll Health System’s vision of excellence and integrated care delivery. The staff nurse complies within the established standards of practice, policies and clinical guidelines reflective of evidence-based best practice. The practice of the staff nurse contributes to creating an environment that is patient focused, family centered and is designed to provide continuation of care and patient transition management. The Cardiac Center Care Coordinator is a registered nurse (RN) with advanced education or training in the care of pediatric cardiology and cardio-thoracic surgery patients. Working closely with the Cardiologists, Cardiothoracic Surgeons, Advanced Practice Providers and clinical nursing staff, this RN facilitates communication between the members of the health care team and patient/family to enhance the quality of the care provided to patients under the Heart Center Service Line. Working knowledge of EMR systems as it applies to Heart Center patients. 2. Maintain all hospital based compliance initiatives. 3. Maintain confidentiality and adherence to all DCH policies and procedures. 2 Cardiac Center Care Coordinator (updated 10.07.2020) 4. Demonstrates business practices and personal actions that are ethical and adhere to corporate compliance 5. Assess patient and family learning needs throughout hospitalization. 6. Provides teaching to prepare patients/families for interventional procedures when indicated. 7. Coordinate with social work, case management or other services to identify, address and document patient/family needs. 8. Monitor interventional procedure(s), update parents/family as well as clinical/medical staff on status of patient and surgery/intervention. 9. Ensure patient information, clinical pathways/protocols and nursing data collection tools are identified, utilized and reviewed. 10. Participate in bedside education of staff when indicated. 11. Daily participation in multidisciplinary rounds on all Heart Center Service Line patients throughout the hospital. 12. In collaboration with medical team, coordinates discharge planning. Ensures discharge summary is completed and reflects the appropriate discharge plan and follow up. Assist with scheduling of all required ambulatory follow-up appointments. Provides teaching to prepare patients/families for discharge planning. Coordinates ambulatory referrals to the High Risk Clinic. 13. Complete/document phone calls for Society of Thoracic Surgeons (STS) Database compliance/maintenance in EMR. 14. Regular Attendance at care conferences/discharge planning meetings. 15. In collaboration with medical team maintains involvement with high-risk congenital heart disease patients including interstage patients. 16. Participation in Congenital Cardiothoracic Surgery Conference, Cardiac Morbidity & Mortality Conference, CV Protocols and other Department based meetings. 17. Participation in one hospital based QA/QI initiative (yearly). 18. Identify and develop department-based QA/QI initiatives. LICENSE, EDUCATION AND/OR EXPERIENCE:  BSN required.  Current RN license in the State of Texas.  Minimum of 2 years of pediatric experience with cardiac patients in direct patient care, including involvement in teaching and staff guidance.  Excellent communication skills.  Bilingual preferred. 4 Cardiac Center Care Coordinator (updated 10.07.2020) CERTIFICATES, REGISTRATIONS.  American Heart Association Basic Life Support (BLS) Required pre-hire  American Heart Association PALS and ACLS Certifications, preferred.  National Certification in specialty must be attempted within 3 years of completing orientation and continuously maintained ongoing Until all children are well.
