JD Attached* The Wound Ostomy Registered Nurse (RN) is a licensed professional responsible for planning and providing care for patients with complicated wounds, pressure ulcers, and/or any patient who has a potential skin care problem, in order to assist the patient in achieving a maximum level of wellness and independence. The Wound Ostomy RN will consult with other members of the health care team in the management of acute and chronic wounds, and will be actively involved in the management and prevention of pressure ulcers. The Wound Ostomy RN operates within the framework of Driscoll Children's Hospital Mission Statement and Core Values. Exercising appropriate age-specific safety practices when interacting with customers, patients, and families. Considers developmental age and approach to and preparation of child. Positions and handles patient based on an understanding of developmental age. Implements safety measures that are appropriate to developmental age. Uses age appropriate equipment and supplies. Essential Duties and Requirements: Maintains utmost level of confidentiality at all times. Adheres to hospital policies and procedures. Demonstrates business practices and personal actions that are ethical and adhere to corporate compliance and integrity guidelines. Demonstrates expertise in the execution of daily patient care, providing collaboration, direction, and instruction to the staff in the application and utilization of current wound care protocols. Promote a team approach through coordination of the efforts of all members of the Health Care Team Serves as patient advocate. The ultimate goal is to maximize the patient’s return to an optimal level of functioning. Provides safety, comfort care, and treatment of patients according to established legal, ethical, and institutional standards. Serves as a resource to patients, families, and staff. Initiates, evaluates and revises plans of care that reflect a multidisciplinary approach to patient care Coordinates with physician, nursing, and other personnel, patient and family in the meeting of wound care needs of the patient. Asses, implements, and evaluates comforting, therapeutic, restorative, and preventative measures for selected patients. Analyzes selected patient care plans for evidence of documentation of standards of practice, shares findings with staff, and collaborates in developing a variety of valid and reliable assessment tools to assist staff with documentation records. Maintains and teaches others the specialized knowledge and skill necessary to perform techniques and routines in wound care Offers consultative services to provide expertise in the planning and delivery of special patient care problems. Performs, assists with, and/or participates in planning, development, execution and evaluation of clinical research projects. Participates in the Quality Improvement Program; collaborates in setting standards of care and monitors indicators of quality for Wound Ostomy Service Line. Promotes and uses cost-saving routines during procedures and activities. Adheres to internal controls established for department. Maintain current knowledge base on the prevention and treatment of wounds through attendance of educational programs/seminars/conferences reading journals and participating in committee or group programs. Implements treatment plan according to current wound care protocols and explain dressing rationale. Institute preventive measures and monitor results of care delivered. Informs APRN and physicians on status of wound patients. Provide demonstrations/education on dressing techniques, principals of aseptic technique Monitor prevalence and incidence of wound and skin care patients. Monitor for nutritional deficiencies that will impact or impede wound healing. Ensure adequate materials are on hand for every patient. Participates in wound care education designed to enhance the benefits patients can derive from the most recent techniques/procedures available with consultation from the health care team assesses patient condition by reviewing all systems, the patient’s health records and/or history and recommends to the interventions for wound healing. Coordinates health care needs by providing recommendations to subspecialty clinics and community resources. Provides teaching to patients and families on preventive, curative and rehabilitative measurers involved in health care determined by the individualized plan of care. Provides hands on care and teaching to patients who have complex and/or non-healing wounds. Documents all patient education activities and response of patients/families. Assumes responsibility for maintenance of aseptic technique and adequacy of supplies. Assists in the review and authorization of wound ostomy supplies. Evaluates new and currently used products for wound ostomy care. Participates in medical record audits for quality and utilization purposes and compliance. Participates in writing and updating policies, procedures and teaching documents related to complex wound ostomy management. Pre and post-operative counseling for surgical patients. Provides follow up and interventions to patients regarding wound ostomy management concerns. Participates in continuing education regarding wound ostomy care management including but not limited to: 1. Prevention 2. Early Identification/Risk Assessments 3. Causes of Skin Breakdown 4. Wound Ostomy Assessment 5. Wound Ostomy Management Interventions 6. Documentation Until all children are well.
