Department/Unit: Work Shift: Day (United States of America) The Patient Care Assistant (PCA) will work under the direction of the Registered Nurse. During the assigned work period, PCAs will collect patient data, complete personal care of the patient, perform point of care testing such as whole blood glucose, turn & position and/or mobilize patients, and assist with caring for the needs of assigned patients with a focus on progress toward discharge, including during transitions within the acute care stay. The Patient Care Assistant promotes the availability of resources for patient care through collaboration with Materiel’s Coordinators for use, stocking, and storage of supplies. The Patient Care Assistant participates in the creation and maintenance of a clean, orderly, and safe environment of care. PCAs may also be asked to provide assistance with patients requiring 1:1 observation. A. Mission, Core Values and Service Excellence: 1. B. Direct Patient Care 1. Collects pertinent data and information relative to the patient’s health or situation, including vital signs, height and weight, blood glucose, and food and fluid intake and output. 2. Collects specimens such as urine, sputum, and stool, and transports as necessary. 3. Reports abnormal findings to the RN and patient care team. 4. Participates in care planning and the nursing report process. 5. Incorporates patient/family rights to participate in decision making about their care. 6. Utilizes standardized techniques for keeping patients and families informed. 7. Provides personal care to patients including bathing, oral care, and skin care. 8. Assists the patient with eating and hydration, grooming, dressing, and toileting. 9. Incorporates safe patient handling into basic restorative care such as ambulation, range of motion, positioning, alignment, and use of assistive and prosthetic devices. 10. Employs strategies to promote a clean, orderly, and safe environment. 11. Demonstrates standard and transmission-based precautions and infection control techniques. 12. Transports patients via stretcher, wheelchair, bed, isolette, crib, or escorts to various areas within the Hospital. 13. Transports equipment, supplies, patient belongings, pharmaceuticals and specimens to and from other departments, as requested. 14. Assists with ordering supplies and equipment; maintains designated par levels of supplies in collaboration with Materiel Coordinators. 15. Provides 1:1 patient observation as assigned. 16. Provides details related to patient progress toward the achievement of goals and outcomes to the RN. 17. Receives report of critical laboratory values and relays to the RN in a timely manner. 18. Documents all care in the patient record. C. Patient Safety 1. Supports a system that encourages the identification and analysis of errors and near misses in the context of a safety culture. 2. Participates in departmental safety inspections to assure compliance with hospital safety standards as assigned. 3. Is aware of and compliant with all hospital and regulatory agency safety goals and objectives as well as limitations of unlicensed assistive care.
