Job Description The Community Health Worker (CHW) provides supportive services in the community setting for an assigned panel of enrolled Health Home participants who are defined as high utilizers (frequent emergency department (ED) visits and/or multiple hospitalizations). The CHW works to assist patients in obtaining needed social or community services. The CHW meets with the patient in the community setting to determine barriers to care and facilitates patient engagement in the ambulatory clinics with their Health Home team in order to reduce avoidable hospitalizations and ED use. The CHW works collaboratively with the multidisciplinary Health Home team to assist with medication compliance, hospitalization and ED follow up. Minimum Requirements High school diploma or GED Certificate Certification as Community Health Worker Good verbal communication skills Able to document interactions with client in the EMR Responsible and accountable work ethics Good time management on job assignments Able to work with multicultural staff and clientele Able to work as part of a team Able to work with minimal supervision in community setting Knowledgeable about formal and informal supports available in the consumer’s community Knowledgeable about diverse challenges facing chronically ill patients with limited resources As job requires meeting patients in the community, the CHW must have reliable transportation and must meet the requirements of the TMC Fleet Driving and Safety Compliance policy and must continue to meet these requirements as a condition of employment. Preferred Have worked in a health care setting and/or have personal experience as a caregiver or family member of someone with a chronic illness Evidence of punctuality with good attendance at a previous job Training in de-escalation techniques Working at University Health is about making a difference.
