The Care Coordination Manager acts as a patient advocate/care coordinator to hospital clients. Provides expertise for enhancing the quality of patient management, satisfaction, continuity of care and timely cost effective practices. The manager coordinates, negotiates, procures services and resources for, and manages the care of complex patients. Applies review criteria to determine medical necessity for admission and continued stay. Additionally, performs care coordination and discharge planning functions Assesses readmissions and assists the quality management process by identifying trends and establishing plans of correction where appropriate. The manager of care coordination collaborates with the interdisciplinary staff internal and external to the organization. Participates in quality improvement and evaluation processes related to the management of patient care. Coordinates the integration of the social service function into patient care. Assists patients in understanding their conditions and available medical treatments. Works with affiliated agencies, such as counselors, and acts as an advocate between service recipients and these agencies. Designs health plans for patients, which contain activities from administering medication to consulting with other healthcare personnel. Collaborates with other professionals to evaluate patients' medical or physical conditions. Identifies environmental impediments to client or patient progress through interviews and review of patient records. Monitors, evaluates, and records client progress, according to measurable goals described in treatment and care plan. Ensures compliance with quality patient care and applicable regulations. Utilizes consultation data and social work experience to plan and coordinate client or patient care and rehabilitation. Establishes and continuously assesses the effectiveness of the internal controls within the unit and compliance with University policies and procedures. Ensures employees are trained on controls within the function and on University policy and procedures. MINIMUM QUALIFICATIONS: Education: Bachelor’s degree in a relevant field Experience: Minimum 3 years of relevant experience Knowledge, Skills and Attitudes: Knowledge of business and management principles. Ability to direct, manage, implement, and evaluate department operations. Ability to establish department goals, and objectives that support the strategic plan. Ability to effectively plan, delegate and/or supervise the work of others. Ability to lead, motivate, develop and train others.
