Responsibilities The Rehab Services Supervisor will provide both supervision and direct patient care within the scope of their license for patients requiring therapy services according to principles and practices of the profession and A.O. Fox Hospital policies and procedures. Rehabilitation Services Personnel Supervision: The Rehab Services Supervisor is responsible for the direct supervision of rehabilitation services providers at A.O. Fox Hospital and Nursing Home, including Physical Therapists, Occupational Therapists, Physical Therapy Assistants, Occupational Therapy Assistants, Speech Language Pathologists, Rehabilitation Aides, and per diem clinical staff. Ensures that these employees know what is to be accomplished in their jobs; whether their performance is satisfactory and what they can do to improve performance. Participate in the orientation and training of new hires for the facility. The Rehab Services Supervisor ensures that these employees possess or are developing the necessary knowledge and skills to perform their work. The Rehab Services Supervisor ensures that staff have successfully completed and maintained assigned competencies, mandatory education, and health requirements by role. The Rehab Services Supervisor assists management with completion of annual appraisals on time per policy and follows Human Resource guidelines for disciplinary actions and performance improvement plans. In the absence of the Manager, or upon delegation, makes time-off request and staffing allocation/coverage decisions in order to optimize access and prevent appointment cancellations to the greatest degree possible. The Rehab Services Supervisor establishes timely and effective communication with direct reports to keep them aware of important corporate, divisional or departmental information and to receive information and insights from employees. Delegates clinical staff to act as clinical education instructor for students who come from a variety of settings including independent or school-arranged student observations, New Visions, PTA, and PT programs Accountability: Accepts responsibility for all aspects of leadership without blame, complaining or procrastination. Assists Manager in ensuring compliance with all regulatory agency requirements (e.g. The Joint Commission). Proactively identifies potential and actual risks to safety, identifies the underlying causes and makes necessary improvements in order to mitigate risk. Maintains up to date emergency and life safety plans for rehab services, in collaboration with appropriate organizational staff, and conducts and documents related drills, trainings, and logs per organizational policies. Maintains clinical competency and actively participates in in-service training programs. Maintain and monitor quality and performance improvement indicators, including staff productivity, patient volumes, financial stewardship, and other quality improvement metrics. Interdisciplinary communication/collaboration: Promotes collaboration between inpatient case managers, providers, MDS coordinator, social workers, administrative staff, and clinical staff to support timely and appropriate consultations and treatment by the rehab team. Addresses interdisciplinary concerns regarding rehabilitation services access and care in a positive and collaborative manner. Attend (or delegate as appropriate) care conferences for all initial, annual or therapy program assessments, as well as all sub-acute care conferences. Acts on behalf of the Manager, in his/her absence, including issues regarding professional practice, hospital policy and procedure, and work schedules. Attends meetings as requested by the Manager, representing the needs of the Rehab Services program. Essential Clinical Functions: Evaluates impairment and/ or disability and develops a treatment plan to improve function. Documents in Epic Unity the treatment plan in the EHR of acute care patients on the same day that the patient is evaluated, 100% of the time, with no more than 1 exception per month (based on a review of 25 inpatient charts). Documents in Epic Unity using standardized format to assess the patient’s current status, on the same day the patient is seen. Completes all required documentation in nethealth and sigmacare for therapy assessments, treatments, screens, and/or service requests provided in the SNF setting. Continuously seeks opportunities to both learn and teach clinical techniques that will enhance patient care. Actively participates in clinical in-services as a presenter and attendee. Demonstrates the ability to evaluate and/or treat the assigned number of patients per day, and determine the need for follow-up visits to maximize the functional outcomes. Maintains schedule to provide at least 0.5 clinical productivity with respect to staff-level benchmarks. Responds to variable demands and patient needs to ensure timely access for acute and established patients for short term, long term, and outpatient rehab. This may require changes in work assignment and /or extended hours. Accepts responsibility for reviewing patient referrals and insurance information to insure coverage for rendered services is appropriate, and within the acceptable authorization guidelines, 100% of the time. Review SNF documentation for timely and appropriate referrals, re-certifications, properly dated notes, accurate CPT codes, and adequate justification of need for medically necessary therapy services. Review the insurance coverage of each patient. Accurate and timely completion of MDS/PDPM sections related to therapy. Qualifications Bassett Healthcare Network follows the New York State and New York State Department of Health COVID-19 vaccine mandates for healthcare workers. Education/Qualifications: Bachelor’s degree in one of the following: Physical therapy. Occupational therapy. Speech pathology. Speech Pathologist. 1 year of supervisory experience is preferred or evidence of completion of management training courses. Licensure/Certificates: Current NYS license in one of the following is required (or eligibility for same): Physical Therapist. Occupational Therapist. Speech Pathologist. Skills: Must be capable of understanding and carrying out prescribed therapy services as requested by physicians and/or midlevel providers. Must be able to direct Rehabilitation Services staff in matters related to departmental and hospital policy and procedure. Must be available to discuss and resolve issues regarding the delivery of services for both acute care and SNF patients. Must be capable of working collaboratively with outpatient coordinator and departmental management to meet patient care need and staffing levels across the entire department. Physical Requirements: This position requires frequent standing, sitting, and walking, as well as occasional lifting, kneeling, and bending. The ability to transfer patients to various surfaces is also required.
