Role and Responsibilities
The Time Limited Subsidy (TLS) program provides housing for those experiencing homelessness referred from temporary housing programs. This program aims to provide case management, self-sufficiency, connection to services, and time-limited subsidies for participants. Under the leadership of the Health Intervention Program Coordinator, this position is primarily responsible for accessing thorough knowledge of the Housing First model
- Conducts comprehensive screenings and assessments with individuals (or families) to collect functional, environmental, psycho-social, financial, employment, housing, educational, and health information, as appropriate assess barriers facing the clients and develop a case plan.
- Coordinate individualized planning with clients to meet short- and long-term needs.
- Provide service linkages and support systems to ensure identified needs are met.
- Coordinates case conferences with residents, consults with service team, monitors progress of clients on case plan, assists clients to meet the agreed upon goals.
- Provide a range of supportive services (service planning; housing and rental assistance, housing search and location, linkages to health, mental health, substance use disorder services, and legal services, benefits establishment, transportation, basic needs (food, clothing, hygiene), crisis management, eviction prevention, life skills etc.) transitioning from homelessness to permanent housing.
- Conduct monthly home visits, must meet with client’s face-to-face no less than once a month
- Update individual service plan no less than 1x every 3 months (maybe sooner depending on how soon the goals are accomplished)
- Responsible for providing case management to a caseload of 20 clients.
- Provide a range of supportive services (service planning; housing and rental assistance, housing search and location, linkages to health, mental health, substance use disorder services, and legal services, benefits establishment, transportation, basic needs (food, clothing, hygiene), crisis management, eviction prevention, life skills etc. Transitioning from homelessness to permanent housing.
- Immediate needs assessments, and crisis stabilization as a member of the program case management team
- Provides assistance with permanent housing placement for participants
- Collaborate effectively with all LAHSA staff, HACLA, LACDA and property managers and landlords
- Other duties that may arise to support the program mission and goals
- Assigned tasks for periods up to 5 hours per day. Duties may occasionally include lifting up to 30 pounds
- This is a field-based job requiring frequent transportation to various sites, transportation of supplies and use of personal vehicle.
- Additional duties as assigned
- Regular work schedule may need to be adjusted for specific situations as dictated by program needs.
Qualifications and Education Requirements
- Bachelor’s Degree (Social Work, Social Services, public administration, public health or related field) OR A combination of education and relevant experience (two years’ total)
- Working knowledge of Microsoft Suite, computer literacy and willingness to learn other applications
- Good communication typing skills and computer proficiency, including Microsoft Office Suite.
- Familiar with HMIS and Clarity electronic reporting and navigation system
- Experience performing intakes and assessing social service needs
- Experience working with persons with co-occurring disorders and people experiencing homelessness
Preferred Skills
- Bilingual or bicultural capabilities with Los Angeles County threshold languages: Arabic, Armenian, Cantonese, Farsi, Urdu, Khmer (Cambodian), Korean, Mandarin, Russian, Spanish, Tagalog, Vietnamese, and other Chinese languages.
General Requirements and Knowledge, Skills, and Abilities (KSA’s):
● Permanent resident or U.S. Citizen; proof of work eligibility upon hiring.
● Must have three (3) years of recovery or drug free lifestyle as of the date of the application.
● Valid California Driver's License and current automobile liability insurance.
● Must provide proof of full vaccination and 1st Booster against Covid-19 upon hiring.
● Ability to get along well with others, be punctual, pleasant, and tactful with professional appearance and conduct.

Physical Requirements:
Moderate Duty
This class requires that the incumbent stand or walk most of the time with bending, stooping, squatting, twisting reaching, working or irregular surfaces, occasional lifting of objects over 25 pounds, and frequent lifting of 10-25 pounds. Ascending and descending ladders and staircase will be more frequent. An example is this position frequently will be moving and organizing agency supplies.
