Job Summary

The LPN works under the direction of a registered nurse and utilizes the nursing process in the planning, implementing, and evaluating of nursing care to the patient in their home. The LPN may collect data on the health status of the patient in conjunction with other members of the health care team for the identification of health goals. The LPN may communicate with the patient the status of goal achievement as a basis for reassessment, recording of priorities and new goal setting for contribution to the modification of the nursing care plan. He/she ensures that compassionate, engaged care meets or exceeds quality standards. Care is provided in an efficient and safe manner consistent with the South Carolina licensure and AAMC/HRHC standards of care.

Minimum Education Requirement

Graduate of an LPN program.

Minimum Experience Requirement
-
Possesses a thorough knowledge of nursing principles as well as hospital policies and procedures; demonstrates ability to provide quality patient care; communicates and works effectively with people; possesses ability to assess patient's needs, ie. clinical, teaching, mental and spiritual.

Minimum License Requirement
-
License to practice as Licensed Practical Nurse in the State of South Carolina or have a temporary permit to work within SC. BLS Certification prior to having patient care. Current SC driver's license, valid car insurance and must maintain a driving record that is insurable under AAMC's auto insurance policy.

Physical Requirement/Working Conditions

May have irregular hours and long periods of standing or sitting. Should be able to lift up to 50 pounds, and will seek assistance when heavier objects need to be lifted. Possible exposure to communicable diseases, body fluids, toxic substances, medicinal preparations, and other conditions common to a healthcare environment. May be exposed to infectious specimens, diseases, soiled laundry, lifting and handling of heavy patients, but potential for personal harm or injury to self or others is limited when proper safety and health precautions are followed. External applicants, as well as position incumbents who become disabled, must be able to perform the essential functions, either unaided or with the assistance of a reasonable accommodation to be determined on a case by case basis.
