Summary
Under the supervision of the Administrator:

Responsible for all admissions and discharge policies and procedures

Travel to residents or hospitals to meet potential clients and source leads
- Coordinate resident rooms prior to admissions
- Represent the care center as the in-house point of contact for hospital and family members regarding admissions
- Transfer information to and from electronic health record (EHR) system
- Initiate insurance verification with ABCM Centralized Billing as required
- Complete Admission Financial Application (AFA) with potential resident or responsible party
- Ensure COVID-19 assessment is completed
- Enter and track AL and NF hospitalization/readmissions in Point Click Care
- Coordinate discharges from the care center
- Other duties as assigned
Qualifications / Requirements

Must have an unrestricted and active Iowa (or compact state) RN or LPN license and up-to-date CPR certification
- Skilled nursing / long-term care (LTC) / rehabilitation nursing experience preferred, with knowledge and understanding of Federal and State nursing facility rules and regulations
- If licensed as an LPN, the Iowa Board of Nursing requires the completion of a Board-approved supervision course within six (6) months of working in a charge / supervisory capacity
- Must have excellent interpersonal and organizational skills
- Must have excellent oral and written communication skills specific to the English language

Shift: 8a-4p, Flexible Scheduling
Base Wage: $34.00/hr (RN) and $30.00/hr (LPN), Negotiable (DOE)
Wage Range: $30.00/hr-$39.50/hr, Negotiable (DOE)
2nd Shift Differential: $2.00/hr
Weekend Differential: $2.00/hr
Years of Experience Incentive Pay: $0.15/per year, up to ten (10) years
