Job Purpose Summary:
- Responsibilities include resource assessment to identify client needs and establish connections/rapport within the community to provide myriad resources and connection to services. Case managers will be responsible for service planning and service plan implementation & coordination, as well as monitoring and follow-up on these services. Case manager is expected to be active in aftercare planning, crisis intervention and third-party requests.
Key Performance Indicators Include:
- Documentation of case management services within 24 hours.
- Aftercare Planning coordinated, completed, and documented for every client
- Chart Auditing is conducted monthly by Case Management Team Lead to ensure efficiency of following clinical notation standards. Chart Audit score meets minimum of 3.3 requirement.
Job Responsibilities:
- General Case Management functions supporting client needs.
- Collaborating with Clinical Supervisor on important collateral needs.
- Attend daily huddle meeting to coordinate daily case management needs
- Coordinate/facilitate Third Party records requests.
- Coordinate ancillary services
- Participate in weekly Treatment Team Meetings.
- Assist clients with securing resources.
- Discharge & Aftercare planning.
- Maintain communication with families, patients, and staff.
- Initiate and/or follow up on existing FMLA and Short Term Disability Claims
- Assist with Unemployment if applicable
- Assist with Cobra enrollment if applicable

Job Type: Full-time
Required education: Associates Degree or college coursework related to this position, Bachelor’s Degree Preferred.
Required experience: 2-4 years’ experience working in a Drug & Alcohol Treatment Facility.
Proper Knowledge and Skills:
- Proper English and grammar, usage and spelling
- Software application related to the field
- General offices principles and practices
- Basic computer knowledge including email etiquette
- Professional phone and customer service demeanor
- Microsoft Word, SharePoint, Excel, and Outlook
Physical requirements:
- Must be able to lift 20-50 lbs.
- Occasional to frequent walking, sitting, standing, driving
- Close eye work (computers, typing, reading, writing)
