Able Home Care is looking for Part-time or Per Diem Home Health Care Nurses in the Plymouth, MA area. There is a potential for this position to become Full-time.
Responsibilities:
- Delivers professional nursing services in the client's home setting.
- Key mission is to educate clients on disease process which includes managing medications and creating Oasis documentation.
- Prepares clinical and progress notes, submitting all required paperwork daily.
- Continuing professional development offered in learning Oasis.
- Coordinates services, informing the physician, management, and other personnel of changes in the client's condition and needs.
- Ensures quality and accuracy of patient records and charts in compliance with Medicare and Mass Health standards.
