Overview

The AbleTo Assessment Coordinator directly reports to the Supervisor of Clinical Risk and Patient Safety. They support the nationwide network of licensed clinicians in making program enrollment eligibility determinations based on the review of a comprehensive initial assessment and AbleTo’s exclusion criteria, as well as serving as primary consultant for complex and risk related participant presentations when the assessment delays for further review. The Assessment Coordinator also serves as the primary responder to incoming escalations from the Urgent Clinical Hotline that serves the AbleTo provider community, Engagement Center, and the Service Center by providing real time risk assessment and participant eligibility determinations, as well as crisis intervention and care coordination for prospective participants. The ideal candidate will have risk assessment, crisis intervention and triage experience, and have ability to provide clinical consultation to licensed clinicians while possibly managing multiple complex tasks that may have time sensitive patient safety circumstances.

 

What You’ll Do: 

  • Complete clinical review of initial assessments conducted by AbleTo licensed Therapists to ensure that eligibility and exclusionary criteria are considered. 
  • Determine program enrollment dispositions and treatment recommendations based on assessment outcomes.  
  • Provide clinical consultation to AbleTo licensed Therapists that reflects clinical best practice regarding risk, treatment planning, and care coordination, and targets the participant’s primary complaints and identified needs.  
  • Ensure documentation is thorough and clearly defines eligibility concerns and recommendations, and aligns with quality requirements for re-engagement and follow up with the participant. 
  • Provide clinical support and when needed, network training to ensure initial assessments requiring review are conducted in compliance with AbleTo standards. 
  • Develop strong and collaborative consultative relationships with AbleTo licensed Therapists, by providing concrete guidance and feedback with regards to their assessment, intervention and risk management activities.
  • Liaise with the AbleTo Sr. Clinical Team to provide feedback about assessment processes as well as feedback about any provider performance concerns. 
  • Act as a primary responder to the Ableto Urgent Clinical Hotline receiving calls from the Engagement Center, Service Center, nationwide network of licensed clinicians and behavioral coaches, as well as the coaching community. 
  • Conduct risk assessment, crisis intervention, triage and any care coordination necessary following a clinical escalation from the Urgent Clinical Hotline.
  • Provide crisis consultation to AbleTo team members who contact the Urgent Clinical Hotline. 
  • Act as a liaison between the Engagement Center and the Sr. Clinical Team.
  • Engage in required timely workflows, documentation, volume tracking, and interventions.
  • Attend routine oversight meetings with the assigned Supervisor of Clinical Risk and Patient Safety, as well as the customary weekly, monthly and quarterly Sr. Clinical Team Meetings.
  • Participate in the development and delivery of training to the Engagement Center (non-clinical) staff.
  • Coordinate with referrers when necessary to clarify eligibility criteria for referrals made to AbleTo, and to ensure clinical recommendations for members redirected to a different level of care are clear and collaborative.
  • Work closely with the Sr. Clinical team to implement the Company’s overall care coordination efforts and quality management protocols.  

 

Who You Are:  

  • Master degree of Social Work or equivalent degree (independent license required: LCSW, LPC, LMHC, etc.)
  • Minimum of 3 - 5 years post master degree required
  • Clinical practice experience that includes conducting mental health assessments with adults, performing crisis assessment and intervention, and engaging in triage and care coordination
  • Supervisory experience a plus  
  • Ability to provide clinical consultation regarding risk assessment and crisis intervention to licensed clinicians within a national tele behavioral health network
  • Knowledge of medical and behavioral health conditions and comorbidities
  • Ability to act as a liaison between AbleTo’s clinical team, Engagement Center, Service Center and coaching community.
  • Ability to collaborate interdepartmentally
  • Ability to self manage in a remote position 
  • Position is full-time, five days per week with a minimum of one permanent weekend day (e.g., Tuesday - Saturday; Wednesday - Sunday; etc.)
