Role Description
This is a full-time field position for a Case Manager located in Bronx, NY. We have 18 apartments throughout the Bronx-home visits are required. The Case Manager will be responsible for assessing client needs, developing care plans, coordinating services, and monitoring progress towards goals. They will also collaborate with interdisciplinary teams, provide crisis intervention, and maintain accurate documentation.
Qualifications
- Case management, client assessment, and care planning skills
- Ability to coordinate services and monitor progress
- Strong interpersonal communication and crisis intervention skills
- Experience in maintaining accurate and confidential documentation
- Knowledge of community resources and social service programs
- Bilingual proficiency in English and Spanish is a plus
- Bachelor's degree preferred in Social Work, Psychology, or related field
