Title: Intake Coordinator
Job Summary:
* Primary function is to facilitate the referral intake process, coordinate care with interdisciplinary team, the patient/client their family, and the referral source.
Job Qualifications:
*One year or more in providing coordination support preferred. Graduation from a standard senior high school or equivalent. Some college experience preferred but not required.
*If at least 18 years of age, have proof of competency through education and/or experience or demonstrate competency to perform tasks as assigned. If under age 18, must successfully demonstrate competency to perform tasks assigned.
* Good interpersonal skills.
* Reliable transportation.
*Background checks must agree to and pass a criminal history check and an Employee Misconduct Registry check.
Environmental and Working Conditions:
*Works in a routine office environment noise level may be moderately high; must have ability to work flexible schedule and extended hours. Possible exposure to toxic materials, toner, etc.
Physical and Mental Effort:
*Require working under some stressful conditions to meet deadlines and patient/client’s needs, and to make quick decisions. Requires excellent problem-solving skills.
Essential Functions:
*Receives and appropriately documents new referrals in a timely manner
*Participates in coordinating care with Nursing, Supervisors, patients/clients, and the interdisciplinary team.
*Provides effective communication to patients / clients, staff members, other health care professionals and referral source.
*Responds to agency and patient/client needs in a professional manner and creative manner.
*Demonstrates commitment, professional growth, and competency.
*Participates in Agency-sponsored in-service training.
*Promotes the Agency philosophy and administrative policies to ensure quality of care.
