Responsibilities
Hospice Nurse Responsibilities:
- Assess patient and family for physical, psychological, social and spiritual needs and collaborates with patients and families, physician, and Interdisciplinary Group (IDG) to establish the plan of care, implement interventions and/or evaluate care
Initiate and maintain communication with attending physician, Medical Director, IDG, Team Director (TD), and IDG and other agencies to coordinate optimal care and use of resources for the patient and family
Document according to protocol and maintain up-to-date patient records. Ensure plan of care problems, interventions, actions and goals reflect baseline, current, and ongoing status, and changes as they occur - Participate in On-Call or Admissions assignment/rotation, as directed. Provide education to patient and caregiver(s) as needed, based on knowledge needs and learning assessment, patient and family condition, in keeping with plan of care. Coordinate care with IDG based on the determined Plan of Care. Participate in orientation and ongoing education programs, on committees and special projects and other related duties as assigned
- If serving as a Care Manager; Proactively manage using the Case Management Recipe to coordinate all patient care activities in collaboration with IDG. Scheduling patient visits in keeping with needs, physician collaboration and orders management, medication and orders management, medication management, plan of care coordination, plan of care coordination, etc. Supervise Licensed Practical Nurses, Licensed Vocational Nurses and Hospice Aides per protocol. Participate in IDG Meetings, facility patient conferences and other patient and family meetings in coordination with IDG and/or Team Director
- If serving in an On-Call capacity: Collaborate with Administrator On-Cal (AOC)l and cAll Center staff. Perform Admissions, GIP, CC and routine visits, as assigned. Respond to Triage Nurse to receive patient status information and coordinate follow up including visit activity, coordinating medication refills and physician orders and phone support. Coordinate schedule and care activities with AOC as necessary. May participate in IDG Meetings, facilitate patient conferences, and other patient and families meetings in coordination with IDG and/or Team Director. Qualifications
Hospice Nurse Qualifications:
- Associates degree, preferred
- Registered Nurse license required in state(s) practicing
- Previous experience in hospice, home health, general medical surgical or patient population specific (i.e.: geriatrics, pediatric, etc.) nursing.
- Must have reliable automobile, valid driver's license, and the minimum state required liability auto insurance
Meet the regulations and requirements of the State in which the program provides services (i.e.: Current and valid CPR certification, preferred - required in Georgia. Minimum one year of experience, preferred - required in California)
