Position SummaryThe Outreach Coordinator is responsible for community outreach and conducts educational workshops with community partners, churches, schools. The position works to identify new partners throughout the Chicago Metropolitan area. The outreach and educational workshops are delivered to targeted audiences based on programs. In addition to the outreach and workshops, this position links eligible and uninsured patients to preventive screenings. Core Job ResponsibilitiesIdentify and cultivate relationships with community partners including but not limited to local schools, churches and social service and community organizations.Identify, facilitate, and participate in community events such as health fairs, school functions, workshops, festivals, wellness events and other community-related events to link consumers back to ACCESS for their medical care and promote ACCESS within our service areas.Perform street level community outreach in target communities to promote specific ACCESS programs.Develop and implement community outreach activities geared at increasing knowledge within the community about the specific program.Provide education to targeted communities about the importance of healthy life styles, prenatal care, testing for sexually transmitted infections, having a medical home, as well as other important health promotion messages.Screen, educate, answer questions, and assist consumers with applying for ACCESS’ program(s).Support new and existing patients by scheduling health visits at an appropriate ACCESS health center.Support outreach events, referral systems, and programs facilitated by other departments within ACCESS.Maintain detailed record of activities to regularly track impact of outreach efforts and to facilitate appropriate reports to funders and government agencies.Assist in the development of specific marketing and education materials. Requirements/PreferencesHigh School Diploma or GED equivalent required; associates or bachelor’s degree preferred.Spanish-speaking skills strongly preferred; for some programs/grants this may be required.Prior experience in community organizing and/or performing outreach at the community level, preferred.Ability to work a flexible schedule; some evenings and weekends will be required.Intermediate proficiency with Microsoft Office products (Outlook, Word, Excel, PowerPoint)
