Job Description
Fast paced community mental health agency now hiring mental health case managers to provide community psychiatric supportive treatment (CPST) service. While most will work will be conducted in the community, some client services to be provided out of Dayton office location. Working with the Clients in the community, helping them to achieve independent living skills. Full and part time positions available. Competitive pay.
Requirements
- High school diploma required, Social Work Assistant (SWA) registration OR completed associates or bachelor's degree.
- Drivers License
- Current automobile liability insurance
- CPR and First Aid certifications
- Clean Ohio BCI and FBI criminal background check reports
Responsibilities: Position requires strong attention to detail and excellent documentation, time management, and multitasking skills. Job duties include creating and implementing treatment plans, maintaining client stability in the community, client linkage to resources, advocacy, and more:
- Conduct and document client intake appointment
- Work with client to develop and document Individualized Service Plan within thirty (30) days of intake appointment
- Work with client to update and document Individualized Service Plan annually, at minimum
- Provide and document community psychiatric supportive treatment (CPST) service
- Obtain updated consents on an annual basis
