Main Function:
With minimal supervision, as part of a leadership team, and in keeping with Agency policies and procedures and professional standards, and applicable regulations, use own experience and judgement to carry out the mission of the Article 16 and 28 Clinic programs. Responsibility includes oversight of assigned personnel, program budgets, provision of services, and development of new programs and growth of current programs. Contact with public is moderate
Duties:
1. Monitor the provision of services for participants
- Provide oversight to coordinate program services inclusive of following relevant state guidelines and best practices for service delivery
- Communicate regularly with participants/members and their family/advocate to monitor general satisfaction and be available for any concerns to be addressed
- In conjunction with Clinic staff, ensure the development and implementation of a Quality Enhancement program for periodic monitoring of required documentation and quality of services
- In conjunction with Clinician Supervisor, ensure clinician’s caseloads are at maximum capacity
- Monitor the development and implementation of program standards and procedures and ensure that programs’ safety and behavioral intervention procedures align with best practices
2. Perform Treatment Coordinator Responsibilities (Article 16 Clinic)
- Ensure that participants adhere to Article 16 regulations including TB testing
- Ensure that treatment goals are developed for each participant and that goals between disciplines/services are not duplicated
- Interact with referring entities, including physician offices, care coordinators, providers, families and participants
- Review and sign off on each Plan of Care every 6 months
- Act as Liaison with Medical Director related to Article 16 clinic services
3. Manage program expenses in keeping with budget guidelines
- Understand departmental budgets and assist in its development
- Review monthly financial reports and work with the Chief Operating Officer and the Finance Director to modify budget and/or expenses as needed
- Understand current billing and funding requirements and regulations and work with Finance Director to modify practices and spending in order to maximize rates/billing
- Submit proposals for additional funding/grants as opportunity arises
- Prepare program reports and required fiscal data as needed
- Ensure that participants’ rights are protected
4. Maintain appropriate records and prepare necessary reports
- Ensure that all required documentation is completed and maintained per agency standards
- Complete required reports as directed by supervisor
- In conjunction with Clinic supervisor, ensure follow up to incident recommendations are completed in a timely manner as needed
- Review results of participant/member satisfaction surveys and implement changes as needed
- Review external internal QE audit reports and ensure deficiencies are addressed. Monitor and follow up on audit trends
- Implement system to measure quality initiatives and program outcomes. Monitor the status of Program Outcomes data and Agency dashboard.
5. Supervision of assigned staff
- Supervise assigned Clinical staff
- Implement a system for ongoing supervision meetings and department staff meetings
- Monitor the implementation of an orientation and ongoing training program for the employees assigned to the Clinic programs
- Ensure assigned management staff follow agency supervision and evaluation procedures
- Provide ongoing direction, support and performance evaluation of assigned staff
- Document performance concerns and follow up with disciplinary action as needed Work in conjunction with HR as needed
6. Community and Teamwork
- Establish and maintain rapport with appropriate contacts and groups related to health care, including CP Clinic group and Behavioral Health Collaborative
- Develop and maintain relationships with area physician groups, hospitals and other health care entities
- Establish and maintain internal and external communication mechanisms and contacts needed to facilitate Article 16 and 28 Clinic program operations
- Participate in Agency and community meetings, committees and task teams as identified by supervisor
- Serve as member of Senior Management and Leadership Team
- Serve as a member of the Quality Assurance Program Advisory committee and the Agency Safety Committee
- Chairs the Agency’s Infection Control Committee and Clinic Quality Assurance committee
- Encourage linkages between assigned programs, other Agency services and community service providers
- In conjunction with Chief Operating Officer, pursue opportunities to expand and develop services to meet the needs of individuals requesting or receiving services
7. Follow Agency standards and procedures
- Follows and ensures department staff are trained and follow agency standards and procedures
- Maintains compliance and ensures department staff maintain compliance with applicable rules and regulations
- Provides and ensures department staff provide person centered, quality services
- Collaborate with Facilities department as needed, to ensure that Clinic treatment areas meet OPWDD and DOH certification rules and regulations
- In conjunction with the Quality Enhancement department, develops and implements an internal quality enhancement program
8. Other duties as assigned
Education & Experience:
- Bachelor’s degree in a Clinical discipline, healthcare administration, or human service field, and 2 years of clinical experience with at least one year working with people with disabilities required.
- 2 years supervisory experience required.
- Position requires good verbal and written communication skills.
- Public speaking experience preferred.
- Must be able to demonstrate teamwork and professionalism.
- Basic word processing and e-mail skills are required.
- Knowledge of Article 16 and 28 Clinic regulations preferred.
Conditions of Employment:
- Must possess a valid NYS Driver’s license and have transportation necessary to meet departmental needs.
- Must have flexible schedule able to accommodate occasional early morning, evening or weekend commitments.
- Must participate and meet established requirements in a database check through the Division of Criminal Justice Services as defined by the State of New York, Office of People With Developmental Disabilities, Chapter 575 of the Laws of 2004.
- Must participate in successful database checks through the State Central Registry of Child Abuse and Maltreatment, HHS Office of Inspector General, Excluded Parties List System, New York Office of Medicaid Inspector General and Specially Designated Nationals.
