Case Manager for Miami Location
1 year of experience working in the healthcare field as a case manager, a social service worker, or a bachelor’s degree in a comparable field of health care.
1 year of customer service experience.
CASE MANAGEMENT:
Determine program eligibility in accordance with the procedures identified in the BSCIP Policy and Procedures Manual, or as directed by the Department.
Make initial contact with client (consumer)/family within 10 working days of case assignment as required in the BSCIP Policy and Procedures Manual, or as directed by the Department.
Conduct program admission for eligible clients in accordance with the procedures as required in the BSCIP Policy and Procedures Manual, or as directed by the Department.
Develop a Community Reintegration Plan (CRP) for each client in accordance with the procedures as required in the BSCIP Policy and Procedures Manual, or as directed by the Department.
Authorize and monitor services provided to clients with as required in the BSCIP Policy and Procedures Manual, or as directed by the Department.
Maintain documentation of interaction with the consumer/family and service providers as required in the BSCIP Policy and Procedures Manual, or as directed by the Department.
Perform case closures in accordance with the procedures as required in the BSCIP Policy and Procedures Manual, or as directed by the Department.
Adhere to the information security and legal procedures as required in the BSCIP Policy and Procedures Manual, or as directed by the Department.
Meet with Department staff to manage tasks, deliverables, or trainings as directed by the Department.
Travel to perform tasks as directed by the Department.
Perform regular maintenance tasks and complete duties as directed by the Department.
Provide analytical research and complete tasks as directed by the Department.
Correspond (verbally and in written formats) with project teams and internal stakeholders regarding project activities, deliverables, and status as directed by the Department.
Provide detailed accounting of hours worked by tasks monthly, following the Field Accountability Spreadsheet Instructions document, or as directed by the Department.
The Contractor, its employees, subcontractors, and agents must comply at all times with all Department data security procedures and policies in the performance of this scope of work as specified in the Data Security and Confidentially document attached to the purchase order.
