Position Description
Provides case management services to those supported by Achieva and their families. These services include responsibilities in the following functional areas: outcome development, implementation, and progress monitoring; adherence to Frequency and Duration; regulatory and agency compliance mandates. Seeks guidance and support from various other Home and Community Support managers/supervisors and employees as needed. May supervise Home and Community direct support professionals on their caseload.
Job Responsibilities - Highlights
- Actively leads direct support team members in person-centered approaches across the department including both individual and group activities at home and in the community.
- Works with support team members to help establish and implement new goals that support the achievement of a person's person-centered plan and outcomes according to their wants and needs.
- Evaluates and reviews all assessment tools for people they support and ensures completion of any missing information.
- Ensures effective communication and participation in meetings as required with people we support, families, providers and others involved in the provision of supports.
- Monitors delivery of support services by regularly checking in with the person receiving supports, families and team members. Educates families about their service options/choices and may need to assist with finding additional resources if/when requested.
- Identifies areas of concern and/or responds to problems. Ensures to communicate any concerns or issues with team members and supervisor. Assists to resolve issues if/when appropriate.
- If supervising direct support professionals, monitors and evaluates employee performance and takes action to improve or sustain performance as needed. Also completes payroll functions as needed.
- Provides guidance, direction, and support for assigned staff as needed. Collaborates with their supervisor regarding any necessary corrective actions. Follows disciplinary actions as required in conjunction with their Supervisor.
- Remains available to assist, by telephone, in a crisis, at the discretion of the Home and Community Program.
- Prepares documents and/or reviews fiscal and other administrative records as necessary and completes all regulatory documentation that is required.
- Participates on various work teams as requested or appropriate.
- Performs other related duties as assigned. Qualifications
Minimum requirements:
- Equivalent of an Associate's Degree in the social science/human services field and one year of related experience.
- Proven organizational, database, word processing, and computer skills required.
- Valid Pa driver's license and properly insured vehicle.
