SUBMISSION REQUIREMENTS
- 1 year experience administering crisis assessments in BH care or similar setting.
- Masters degree in social work, counseling or psychology required
- Independent licensure as an LICSW or LMHC preferred.
- Hospital approved de-escalation training required
- LICSW - Licensed Independent Clinical Social Worker in MA or LMHC Licensed Mental Health Counselor in MA - REQUIRED AND NON-NEGOTIABLE.
- AT LEAST SPECIFIC INTERVIEW DATES/TIMES OF AVAILABILITY FOR INTERVIEW BE ADDED TO PROFILE - PROFILES WILL NOT BE PRESENTED WITHOUT THIS INFORMATION. "ANYTIME" WILL NOT BE ACCEPTED.

- 1 Supervisory Signed Reference (from within the past two years of employment)
- License/Certifications - Must be uploaded with profile (MSW, LICSW, CPI, BLS, Covid Vaccination proofs (dated on or after 10/3/23) or intent to file exemption form )
