We are seeking a skilled and compassionate Licensed Vocational Nurse to join our healthcare team. Position: LVN / LPN Home Health

Location: Pittsfield, MA 01201

Duration: 13 Weeks Contract

Guaranteed hours: 40

Shift: Day 5x8-Hour (08:00 - 17:00)

Job Details:

Provides coordinates and directs the provision of home nursing care according to physicians orders based on agency policies and procedures through the competent application of the nursing process. Responsible to Clinical Manager Directly Supervises and Evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.

DEGREE OF TRAVEL

Home visits. Periodic office meetings. Must have reliable transportation current driver's license and agency-required liability insurance.'

Average # of patients/day: 5-8

Points per day/week: How are points broken down 30 points per week. Each type of visit and activity (documentation travel etc.) hold a different point value. Contractor will be assigned between 5-10 patients per day that equate to roughly 6-8 points.

Guaranteed Hours: No if there is a situation where the contractor is unable to meet their financial needs due to low patient census please reach out to the AM to discuss floating the contractor to a nearby clinic to pick up hours.

Types of Cases: Start of Care Wound Care Follow up Visits etc.

Years of experience REQ: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience depending on the specialty

Weekend REQ: Varies per location and profession please have traveler discuss during the interview.

Certs REQ: CPR BLS

Territory/County covering: Territory mileage radius Must be willing to travel up to 75 miles from home clinic.

Mileage reimbursed IRS Standard rate. Traveler is compensated from patient home to patient home. The commute to and from the first and last patient are not billable.

Charting: All professions must be proficient with HomeCare HomeBase (HCHB) and used it within the last 6 months. Duties:
- Provide direct patient care under the supervision of registered nurses or physicians
- Administer medications and treatments as prescribed by healthcare professionals
- Monitor patient vital signs and report any significant changes or concerns
- Assist with patient assessments and document observations accurately
- Perform procedures such as catheterization, inhalation therapy, and Botulinum toxin treatment
- Collaborate with interdisciplinary healthcare team members to develop and implement individualized care plans
- Provide education to patients and their families regarding disease management, medication administration, and self-care techniques
- Ensure compliance with infection control protocols and maintain a safe environment for patients and staff
- Adhere to HIPAA regulations to protect patient confidentiality

Experience:
- Minimum of 1 years of experience as a Licensed Vocational Nurse in a Adult Home Health Experience

Skills:
- Excellent clinical skills and knowledge of nursing principles and practices
- Strong attention to detail and ability to multitask in a fast-paced environment
- Effective communication skills to interact with patients, families, and healthcare team members
- Proficient in using electronic medical record systems for documentation purposes
- Ability to prioritize tasks and make sound decisions under pressure
