Job Title: Care Manager (RN)
Job Description
As a Care Manager (RN), you will develop, assess, and facilitate complex care management activities for primarily physical needs members to achieve high-quality, cost-effective healthcare outcomes. This includes creating personalized care plans and providing education for members and their families. You will evaluate the needs of the member, focusing on priorities identified by the member, and recommend or facilitate plans for the best outcomes. You will develop ongoing care plans, collaborate with providers to identify specialists or community resources to address member's unmet needs, and provide appropriate care management interventions. Your role involves coordinating between the member and/or family/caregivers and the care provider team to ensure members receive adequate and appropriate person-centered care. Additionally, you will provide resource support, facilitate care management, perform various types of outreach, maintain compliance with regulations, and assist with backlog referrals. You will be assigned 5-10 referrals daily and will manage a case load of 75 active cases, which are typically 90 days in duration.

Remote, Monday - Friday 8am-5pm, option to start at 7am if needed.
