Licensed Practical Nurses for a leading healthcare system in Long Island, New York!
This position requires an LPN to float between Riverhead, Greenport, and Southampton.
All roles are fulltime with benefit options, 401k, and weekly pay
Location: Riverhead, NY
Description:
This is a fast paced environment and the right person for this position will have to be able to:
- Interviews and maintains patient's medical record to include: nursing observation; health history to include past and present use of medication; vital signs; vision/hearing tests; medications and treatments given; Reactions to tests i.e. PPD measurements; laboratory test results, utilizes patient identifiers. (PEAS, PhQ, Pain, Smoke Status, etc.) CDSS
- Perform in-house labs, collect specimens for out-side labs (ex. Pap, cultures, etc.) perform quality control in accordance with organizational protocols.
- Prepares equipment and assists physician during treatments and examinations of patients.
- Complete and update all assigned tasks & logs. (i.e. correctly recording shipments and vaccines received in appropriate areas, signing out meds & treatments in log).
- Initiates follow-up as necessary (as directed by provider) i.e. encounters, labs, close non-billables, etc.
- Does health teaching as indicated by physician, diagnosis and/or inquiries by patient. Directs patient to RN as necessary.
- Complete Prior Authorizations for specialized medication and services.
- Attend case conferences and document case updates in patient record.
- Collection of urine drug screens.
- Follow up on patient adherence and document accordingly in patient chart.
- Collaborate with community and referral resources
- Participate in quality improvement measures to ensure chart compliance and best practice.

Qualifications:
Must have active NYS LPN license
