What are the responsibilities and job description for the Medical Social Worker II position at Actalent?
Description:
Currently remote - MAY go back into office in Martinez
Schedule: M-F, 8am-430pm, every 8th weekend required
Case Management/ care coordination, particularly for individuals with chronic and/or complex medical conditions who are frequent high utilizers.
Identification, assessment, and resources for mental health conditions such as depression, and anxiety. Ability to identify DTS, DTO, needs for a welfare check, or needs to be evaluated by psychiatry for capacity.
Ability to assess and determine patient/family safety concerns, ability to remain safely at home. Mandated reporting requirements include APS, CPS, CCL, DHS, and Law Enforcement.
Patient navigation, especially for patients moving among different health care levels (e.g., collaboration with inpatient, outpatient, specialty clinic or long-term care).
Identification and referral to specialized services, such as drug and alcohol treatment, legal services, financial and employment counseling, and housing.
Assistance and follow through with Medi-Cal applications, IHSS, transportation, food resources and Probate conservatorship.
Outreach and coordination with other community resources and agencies: and community level advocacy on behalf of patients and families.
Additional Skills & Qualifications:
- CA LCSW Only
- 1 year of LCSW experience w/in the last 3 years
- Case Management Experience
- Must understand Conservatorship process and Custodial Placement
- 2 years of Social Work experience altogether minimum
