To further our mission we are looking for a FFI Case Manager.
Position Overview:
My Place FFI offers transitional housing for youth in foster care ages 18-20 still in CYF. The participants will work closely with the case manager as they will engage in intensive case management services with youth in the My Place FFI Program focusing on helping and guiding the youth achieve self-sufficiency and permanent housing.

Job Responsibilities:
- Developing and maintaining positive relationships with clients in program
- Teach and model independent living skills
- Provide case management duties including school, employment, referrals and contacts
- Maintaining a physically and emotionally safe environment
- Meet with clients in their units, in the office
- Attend and participate in monthly life skills classes with My Place staff and clients
- Help in securing housing for program participants by visiting apartments throughout the Pittsburgh area and ensuring their cleanliness and safety
- Completion of the initial assessment on new clients within program time frames and update assessments as client’s information changes
- Develop, monitor, and revise in conjunction with client, the individual goal plans
- Develop with client, a discharge plan to include provision of two-month aftercare plan services for program graduates
- Maintain and update all clients’ files per regulations under CYF regulations
- Document activities of casework and interaction with clients thought written case notes
- Turn in the program paperwork folder every Monday by 12 noon to the program supervisor.
- Agencies
- Establish/maintain relationships with ILP agencies in Allegheny County.
- Establish/maintain relationships with other service providers as needed to discharge job responsibilities.
- Reports
- Prepare all reports as required by funding agencies.
- Degree in social services and or experience in youth work.
