SUMMARY

Responsible for the coordination and program management of in-Home Services Programs : Community Habilitation, At-Home Respite and, ISS administration.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Supervise the In Home Respite and all Community Habilitation Direct Support Professionals; oversee staff recruitment and hiring, staff scheduling, payroll, and performance evaluations.

Provide or ensure attendance to regular and supplemental training to enhance the professional skills and knowledge of both the In-Home staff and the service recipients.

  • Responsible for department budgeting with supervisor and finance department staff. Approval and submission of CH, In-Home Respite, billing and documentation requirements.
  • Work with the Human Resources Department on the recruitment, assignment and retention of staff.
  • Act as liaison to the Individual, Care Manager, County and OPWDD representatives, Guardians / advocates and staff.
  • Provide supervision to include scheduled and unscheduled visits to observe quality service delivery.
  • Attendance and participation in Life Plan Meetings.
  • Preparation of Community Habilitation plans.
  • Participation in all required training and various related conferences pertinent to CH.
  • Provide financial oversight to individuals in the ISS program.
  • Direct contact with Finance for budget matters concerning current and future recipients of Commhab.
  • Direct contact liaison with Orange County Department of Mental Health and the DDRO.
  • Maintaining all records and Medicaid mandated files per regulations.
  • Performs other duties as assigned or directed.

ADDITIONAL RESPONISBILITIES AS ASSIGNED INCLUDING BUT NOT LIMITED TO :

  • Agency training as assigned
  • Intake screenings
  • Financial assessments
  • Outside committee Involvement

QUALIFICATIONS

SPECIAL REQUIREMENTS FOR THE POSITION

  • Must be able to work a flexible 40 hour week schedule that will include some evening and weekend hours.
  • Must maintain a valid driver’s license that meets agency standards.
  • Must fulfill all mandatory trainings.
  • Obtain criminal background clearance according to OPWDD and agency guidelines.

EDUCATION and / or EXPERIENCE

Bachelor’s degree in human services and 2 years’ experience with population served. Experience supervising or coordinating In-Home services strongly preferred.

LANGUAGE SKILLS

Ability to read, write, speak and understand English. Ability to read, analyze, and interpret reports and documents. Ability to respond to common inquiries, concerns or complaints from employees and customers.

Excellent written and verbal communication skills required.

MATHEMATICAL SKILLS

Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations. Basic understanding of statistics.

REASONING ABILITY

Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of instructions and deal with several abstract and concrete variables.

Ability to resolve conflicts and mediate stressful situations.

PHYSICAL DEMANDS

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

  • Must be physically capable of working with young people and individuals with physical and developmental disabilities.
  • Must be able to drive independently to and from work sites across three counties.
  • Must be SCIP certified and capable of performing SCIP interventions.

Salary Range $25.48 - $28.84 per hour
