Social Services Director
Job Posting for Social Services Director at Addison Heights Health and Rehabilitation
Addison Heights Health and Rehabilitation Center is seeking an experienced Social Services Director. The ideal candidate should be comfortable helping patients adapt to their limitations and assisting families with important decisions about their loved ones.
Social Worker Job duties would include but not limited to:
- Evaluate existing, new and prospective patients based on their needs, desires and limitations.
- Maintain constant communication with residents while addressing their concerns and goals and helping them keep positive attitudes
- Coordinate with therapy department to assist in helping residents feel comfortable and confident.
- Build rapport with residents/patients and their families and relay all concerns to appropriate individuals.
- Develop care plans for residents and provide therapeutic support as needed.
- Ability to make psycho social assessments and develop and implement viable care plans
- Discharge planning
- Excellent listening and communication skills.
- Experience in Long Term Nursing is preferred.
Qualifications
Prefer current license/certification in the Ohio. Must have, as a minimum, two (2) years experience as a Social Worker or related field, in a hospital, nursing care facility, or other related medical facility. One (1) year experience in a long-term preferred. Must demonstrate the ability to assess resident needs, develop and implement a plan of care and coordinate the discharge planning process. Must possess knowledge of accreditation standard and compliance requirements.
