General Purpose:
This position reports directly to the AVP. The Individual works in conjunction AVP to assist in the overall daily function of the program and assure delivery of service in accordance with Goodwill Industries guidelines and the rules and regulation of the Office for People with Developmental Disabilities.
Service focus on Recreation activities, community integration and socialization opportunities
to provide families and caregiver temporary relief of caring for Individuals with Developmental Disabilities. The service allows families and caregivers the opportunity to attend to personal matters-illness, emergencies and complete needed tasks.
Essential Functions:
Observe & supervise Individuals at all times
Ability to communicate effectively with group of Individuals and their families
Complete all required documentations (case notes)required at the end of the shift
Maintain cooperative/ correspondence with families weekly
Audit and maintain Respite file room cabinets in proper order
File Respite forms and miscellaneous documents accurately into charts in chronological order and point out discrepancies AVP
Maintain and ensure HIPAA confidentiality of records.
Follow safety protocols to foster a safe working environment and report unsafe conditions
Ensure all Individuals’ rights are protected
Follow all OPWDD and agencies guidelines and regulations
Clock IN and OUT before and after every shift
Observe Individuals for evidence of injury or bruises and report to AVP
Engage Individuals in community exploration and inclusion by providing them with Person Centered opportunities and daily choice
Follow Medicaid , State funding source and agency guidelines and regulations when providing and documenting services
Qualifications/Basic Job Requirements:
High school diploma or general education degree (GED);
