Are you an LPN looking to make a meaningful impact in a patient’s life while working in a supportive and rewarding environment? Adoration Health is seeking a dedicated Licensed Practical Nurse for a private duty position in Munford. This role involves providing in-home care for a 47-year-old female patient with trach, g-tube, and colostomy care needs.
Key Responsibilities:
- Deliver compassionate and professional nursing care in the patient's home.
- Manage and monitor trach, g-tube, and colostomy care.
- Administer medications and treatments as prescribed.
- Collaborate with the healthcare team to ensure comprehensive patient care.
- Maintain accurate and detailed patient records.
- Provide emotional support and comfort to the patient and their family.
- Day Shifts: We have 2 openings for day shifts.
- Flexible Scheduling: Work with us to find the shifts that best fit your lifestyle.
- Current and valid LPN license.
- Experience with trach, g-tube, and colostomy care preferred.
- Strong clinical skills and attention to detail.
- Excellent communication and interpersonal skills.

- Perform selected nursing procedures within the scope of LPN practice
- Provide patient and family teaching according to plan outlined by the RN
- Monitor patient's vital signs and make specific measurements and observations
- Monitor and record intake and output. Report significant changes immediately
- Observe, assess and record patient's physical and mental condition and reports any changes
- Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordingly
- Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned
- Licensed Practical Nurse / LPN license in good standing
- Previous private duty, home health, hospice, hospital or skilled nursing experience preferred
- Active CPR certification
- Familiarity with electronic health records
- Strong problem solving and critical thinking skills
- Reliable transportation, valid driver's license and current automobile insurance
