Description
At Adoray, the Community Liaison interacts and assists with referrals from healthcare professionals in hospitals, home health agencies, long term care facilities, assisted living and physicians in private practice. The success of this position derives solely from the provider relationships that result in the admission of eligible patients from physicians, discharge planners, case managers, floor nurses, and other healthcare professionals. Represents the organization as a “hospice, palliative, and home health consultant” in the community.
Responsibilities:
Ability to Call Upon Referral Sources
- Meet with appropriate physicians in the assigned territory who influence or make referral decisions to familiarize them with your hospice.
- Meet with appropriate hospital discharge planners and floor nurses to establish a positive referral relationship.
- Meet with appropriate long-term care facility administrative and clinical staff to establish a positive referral relationship.
- Understands or the willingness to learn all possible referral market segments and their value propositions related to the organization.
- Participate in committees and on special projects and other related duties as assigned.
Provides Hospice and Home Health Knowledge
- Coordinates educational in-services and other educational opportunities to referral sources in assigned territory.
- Meet with appropriate referral sources to familiarize them with the patient characteristics and needs that indicate an appropriate hospice referral.
- Develop opportunities to conduct in-service presentations for physician offices, hospitals, nursing homes, assisted living, Community Based Residential Facilities (CBRF) and other healthcare providers.
Service Excellence
- Communicate appropriately with providers, attending physicians, other appropriate staff members, and other agencies as needed to ensure excellent provider relations.
- Work closely with intake, hospice staff, Clinical Directors, and all managers to ensure that promised services are provided.
- Collaborate with Admission/Intake Department and hospice clinical staff to manage intake of patient/families.
Account Development Planning
- Creates Quarterly Growth Plans for the territory and is accountable for the goals of that plan.
- Maintain 100% daily documentation in Customer Relationship Management (CRM), including daily call schedules, weekly call schedules, track of all appointments and activities. Develop, implement, and submit two weeks prior to the next quarter's Community Liaison quarterly market plan.
- Develop and implement a weekly sales call plan that follows quarterly market plan and goals.
- Develop and implement a daily call plan with at least ten sales calls per day that align with quarterly market plan and goals.
- Develop and implement appropriate pre-call plan for each sales call.
- Develop, implement, and work with supervisor on post-call follow-up for each sales call made.
- Develop new accounts to demonstrate month-over-month growth.

Requirements
Minimum Qualifications
- Bachelor’s or Associate's Degree in a related field or five years previous healthcare experience in hospice, home health, healthcare sales or marketing.
- Demonstrate commitment to hospice philosophy of care.
- Demonstrate excellent verbal and written communication skills.
- Demonstrate an ability to work closely with a variety of people while effectively using problem-solving skills.
- Demonstrate an understanding of referral patterns in the service area.
- A willingness to learn Medicare regulations and related reimbursement issues for hospice and long-term care.
- Proficiency/Knowledge of Windows, Excel, and Word.
- Demonstrate a willingness to work as a team player within the department and with the clinical and operational teams.
Preferred Qualifications
- Preferred Candidate Backgrounds Would Include: Sales, Home Care, Hospice, LPN, OT, PT or Social Worker
- Ability to multi-task and meet deadlines.
- Experience with sales and marketing software
Knowledge, Skills, and Abilities
- Maintain confidentiality of sensitive information.
- Participate in hospice orientation and ongoing education programs.
- Demonstrate familiarity with the Hospice Condition of Participation, policies of the agency and state(s), and federal regulations pertaining to hospice services.
- Demonstrate proficiency and willingness with sign-ons and information sessions as assigned.
- Demonstrate 100% compliance with all reporting processes.
- Available for after-hours and weekend inquiry visits and referrals.
- Assist with the “unknown but likely to occur events” that arise related to referral development.
