the Job would be in the Luzerne County and Columbia County. The job functions require driving to each of the group homes within multiple counties. ESSENTIAL DUTIES AND RESPONSIBILITIES: Includes the essential or most important functions and responsibilities. Essential duties are those that must be performed as part of the job.

1. Manages and coordinates Community Living Arrangement (“CLA”)/Community Residential Rehabilitation (“CRR”) programs.

2. Supervises staff in the community homes.

3. Administers day-to-day responsibilities of CLA/CRR programs.

4. Reviews and ensures compliance with the Chapter 6400 and Chapter 5310 regulations pertaining to CLA/CRR homes.

5. Acts as liaison with MH/MR County Supports Coordinator and county and state staff.

6. Coordinates and completes assessments.

7. Provides the assessments as required under the as required under the Chapter 6400 Regulations relating to assessments.

8. Participates in the development of Individual Support Plans (“ISP”), ISP annual updates and ISP revisions.

9. Attends ISP meetings.

10. Fulfils role of plan lead, as applicable under the Chapter 6400 Regulations (relating to development, annual update and revision of the ISP; and ISP review and revisions).

11. Reviews ISP and annual updates and revisions to ISP for accuracy.

12. Reports any ISP content discrepancies to Supports Coordinator, as applicable, and plan team members.

13. Ensures implementation of ISP as written.

14. Supervises, monitors, and evaluates services provided to the individuals in the CLA/CRR.

15. Reviews, signs, and dates the monthly documentation of an individual’s participation and progress toward outcomes.

16. Reports changes related to the individual’s needs to the Supports Coordinator, as applicable, and team members as required under the Chapter 6400 Regulations.

17. Reviews the ISP with the individual as required under the Chapter 6400 Regulations.

18. Documents the review of the ISP as required under the Chapter 6400 Regulations.

19. Provides the documentation of the ISP review to the Supports Coordinator, as applicable, and plan team members as required under the Chapter 6400 Regulations.

20. Informs the plan team members of the option to decline the ISP review documentation as requires under the Chapter 6400 Regulations.

21. Recommends revisions to service or outcomes in the ISP as provided under the Chapter 6400 Regulations.

22. Coordinates the services provided to individuals.

23. Coordinates the training of direct services workers in the content of health and safety needs relevant to each individual.

24. Develops and implements provider services as required under the Chapter 6400 Regulations.

25. Prepares all necessary paperwork as needed (e.g. incident reports, quarterly reports, program specialist checklist, etc.)

26. Coordinates site meetings, appointments for individuals (physicals, dentals, etc), meetings with case management staff and other appointments or meetings as necessary.

27. Offers staff trainings and pre-service training and tracks all staff training.

28. Monitors and addresses on-site issues, including physical condition of the house and yard, petty cash, individual finances, and any other site issues.

29. Prepares for annual licensing and completes documentation of annual licensing inspection instruments (LII’s).

30. Handles fiscal management of site, including program staff.

31. Acts as on-call worker for a one-week period of time approximately every two months.

32. Assists Human Resources Department with recruitment and training of new staff.

33. Assists Human Resources Department with employee issues.

34. Attains certification to instruct CPR/First Aid and Medication Administration classes.

35. Ensures that newly hired staff is fully orientated in their respective program(s).

36. Assists other Program Specialists in ensuring that newly hired staff is fully orientated in their respective program(s).

37. Ensures each RPW/CRRW has accrued at least 24 hours of training per year and facilitates ways to guarantee training commences.

38. Follows HCSIS/Incident Reporting Guidelines

OTHER DUTIES AND RESPONSIBILITIES:

Assists Program Department and Human Resources Department with generalized clerical duties.

Performs other incidental and related duties as required and assigned.

ADDITIONAL SKILLS/ABILITIES NEEDED:

Proficiency in Microsoft Word, Excel, Access, and PowerPoint.

Clear written and verbal communications.

Time management skills.

Excellent attention to detail.

Works well in a team environment.

Ability to maintain confidential information.

SUPERVISORY RESPONSIBILITIES:

Formally supervises staff on an ongoing basis.

EDUCATION and/or EXPERIENCE:

Must have one of the following groups of qualifications:

a. Master’s degree or above from an accredited college or university and one (1) year of work experience working directly with persons with Intellectual Disabilities.

b. Bachelor’s degree from an accredited college or university and two (2) years work experience working directly with persons with Intellectual Disabilities.

c. An associates’ degree or 60 credit hours from an accredited college or university and four (4) years work experience working directly with persons with Intellectual Disabilities

LANGUAGE SKILLS:

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of people.

MATHEMATICAL SKILLS:

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

REASONING ABILITY:

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Job Types: Full-time, Part-time

Pay: $45,000.00 - $55,000.00 per year
