We are an in-home medical group looking for a Nurse Practitioner to see patients post-hospital discharge with the goal of reducing readmission. Patients are based in Volusia County and Brevard. You will be assigned to the market closest to your home.  This role involves travel to each patient location.

Responsibilities:
  • Asses patients and Implement a plan to reduce readmission.
  • Electronic Health Record.
  • Televisit for follow-up care as needed.

Other Details:
  • A full-time schedule is ideal, however, there is flexibility in creating your schedule.   
  • Compensation is based on the reimbursement for each patient that you conduct a home visit with. 
  • This is a 1099 independent contractor role.
  • Must carry your own liability policy.
  • Must be fully licensed APRN, autonomous status preferred.
