Medical Case Manager

*Job Summary* As a Case Manager in our healthcare facility, you will be responsible for coordinating patient care and providing support to individuals in need. You will work closely with healthcare professionals to ensure the best possible outcomes for patients. *Responsibilities* - Collaborate with healthcare providers to develop and implement patient care plans - Monitor and evaluate patient progress throughout their treatment - Provide counseling and support to patients and their families - Coordinate referrals to specialists and other healthcare services - Maintain accurate and up-to-date patient records - Advocate for patients' needs within the healthcare system - Conduct assessments to identify patients' needs and develop individualized care plans *Skills* - Experience in a hospital or clinic setting - Knowledge of behavioral health and acute care services - Ability to work in a fast-paced environment, such as an Emergency Department or Level II Trauma Center - Strong communication and interpersonal skills - Familiarity with outpatient services and patient care coordination - Understanding of Pediatric Intensive Care Unit (PICU) procedures and protocols

Job Types: Full-time, Contract

Pay: $47,713.00 - $90,000.00 per year
