Advantage is currently seeking a PRN Nurse Practitioner to join our vastly growing Hospice Team in the Schuylkill County and Surrounding Areas! This position is responsible for providing patient care in their home.

The Nurse Practitioner is responsible for working in collaboration with the Hospice Medical Director, utilizing advanced knowledge and clinical expertise to provide advanced hospice care services to patients and their families. The Nurse Practitioner is directly accountable for making clinical judgements using independent and interdependent decision making skills.
Responsibilities:
- Follows established standards of nursing practice, federal/state requirements for the position, maintains competence and compliance with requirements in the Medicare Hospice Conditions of Participation and the organization’s policies and procedures
- Performs administrative face to face encounters with hospice patients to identify/monitor existing or potential health problems and appropriateness for hospice recertification
- Documents in the patient’s clinical record according to accepted professional standards and in compliance with organization’s policies and procedures.
- Reports changes in patient’s condition to responsible party and provides necessary interventions as indicated
- Communicates with the Medical Director and other members of the interdisciplinary team as needed
- Applies admission and re-certification criteria to ensure that patients meet hospice requirements and are placed at the appropriate level of care
- Communicates and reports clinical findings to insurance case managers as requested
Qualifications:
- Masters degree in Nursing from an accredited Nurse Practitioner training program
- Current state licensure as a professional nurse in the state of Pennsylvania
- National certification (CRNP) in advanced practice nursing specialty of Adult Health, Gerontology, or Palliative Care from certification body (ANCC, AANP, ONCC, NBCHPN)
- Minimum of one year recent hospice experience preferred. Experience in home care, acute care or long-term skilled nursing care will be considered.
- Certification in Generalist Hospice and Palliative Nursing preferred (CHPN)
- Advanced Certification in Hospice and Palliative Nursing preferred (ACHPN)
