Job Overview:
We are seeking a skilled and compassionate hospice Licensed Vocational Nurse to join our healthcare team. Hospice experienced is preferred. The LVN will see our hospice patin their home, skilled nursing facility, or assisted living facility. You can schedule your visits at your convenience. Very flexible schedule with the option to be paid 1099 or W2. LVN will usually make one visit a week per patient.
As a Licensed Vocational Nurse, you will play a vital role in providing quality patient care and assisting in various medical procedures. This position requires strong clinical skills, attention to detail, and the ability to work effectively in a fast-paced environment.
Duties:
- Provide direct patient care under the supervision of registered nurses or physicians
- Administer medications and treatments as prescribed by healthcare professionals
- Monitor patient vital signs and report any changes or abnormalities
- Assist with patient assessments and document findings accurately
- Collaborate with the healthcare team to develop and implement individualized care plans
- Educate patients and their families on disease management, medications, and self-care techniques
- Perform basic medical procedures such as wound care, catheterization, and specimen collection
- Maintain accurate and up-to-date medical documentation using electronic health record systems (e.g., Cerner)
- Ensure compliance with infection control protocols and maintain a clean and sterile environment
- Assist with the coordination of patient appointments, referrals, and follow-up care
Skills:
- Current state licensure as a Licensed Vocational Nurse (LVN)
- Strong interpersonal skills to effectively communicate with patients, families, and healthcare professionals
- Knowledge of behavioral health principles and techniques for managing patients with mental health conditions
- Proficiency in medical documentation, including familiarity with ICD-9 coding
- Experience with durable medical equipment (DME) usage and maintenance
- Familiarity with sonography procedures and equipment is a plus
- Ability to work independently while following established protocols and guidelines
- Excellent organizational skills to prioritize tasks and manage time effectively
- Attention to detail to ensure accurate documentation and medication administration
- Knowledge of sterile processing techniques for maintaining a safe environment
