Responsibilities:
- To coordinate provision of home health care services to assigned patients that reflect the Agency mission and standards.
- To ensure quality and safe delivery of home health services.
- To ensure that nursing care services are provided in accordance with Federal, State and local standards, guidelines and regulations.
- To demonstrate support for achieving the mission of the Home Health Agency.
Qualifications:
An active LPN license in the state in which practicing.
