Community Care Field Supervisor (Primary Home Care)
AdvantageCare Home Health Inc. is seeking qualified and dedicated individual for the position of Community Care PHC program Field Supervisor. The Field Supervisor is responsible for maintaining and auditing client files, performing annual and supervisory visits, performs orientation and supervision of providers/attendants and clients as required by agency policy and as needed, and periodically assist Coordinators with in-office duties.

The Field Supervisor is responsible for maintaining and auditing client files, performing annual and supervisory visits, performs orientation and supervision of providers/attendants and clients as required by agency policy and as needed, and periodically assist Coordinators with in-office duties.
Candidate MUST have completed two years of full time study at an accredited college/ university or a high school diploma or general equivalency diploma and two years of supervisory experience in a home care setting, current Driver’s License and automobile liability insurance.
PREFERRED: Previous experience with PHC programs and/or management experience
REQUIRED: Skills include a general knowledge of office equipment such as a computer, copier, telephone, etc., ability to provide excellent customer service, have great communication and organizational skills, and admirable leadership qualities.
Job Type: Full-time
Pay: $10.00 - $15.00 per hour
