The role you’ll contribute:

The Care Management Supervisor is under the general leadership of the Care Management Manager or Facility Director and assists in monitoring and performing the daily operations and supervision of team members in the Care Management department.

The Care Management Supervisor is responsible for the daily front line operations as assigned by the Care Management Manager or Facility Director. The Care Management Supervisor will serve as the primary point of contact for Care Managers, Social Workers and patients, to provide mitigation for any barriers/concerns that may arise on the inpatient units. The Care Management Supervisor will be responsible for supporting positive and safe patient outcomes, employee evaluations, and customer experience.

  • Supervises daily operations of discharge planning and social services in the inpatient and emergency department setting as directed by Care Management Manager or Facility Director, including:
    • Direct supervision of Care Management staff
    • Primary point of contact for inpatient unit Care Management related barriers/concerns
    • Manages own assignment and patient/department outcomes of those under direct supervision
  • Assists Care Management Manager/Facility Director to implement the Corporate Care Management strategies
  • Ensures active staff participation in daily Multidisciplinary Rounds
  • Serves as a point of contact for all identified stakeholders
  • Serves as a content resource regarding medical necessity criteria, patient status and discharge criteria
  • Assures staff documentation is completed per guidelines as evidenced by appropriate monitoring techniques and coaches staff as needed
  • Facilitates care conferences for complex transitions and/or placements, as needed
  • Participates in hospital/medical staff meetings to review patients meeting criteria related to high dollar/ extended length of stay and/or difficult placement; seeks assistance from Care Management Manager/Facility Director, as needed
  • Reports pertinent quality/risk issues to appropriate individuals, departments and/or committees in a timely manner
  • Discusses patient, staff, physician, and other department complaints with Care Management Manager/Facility Director and follows up with a written report submitted within 24 hours, as needed
  • Demonstrates a positive problem-solving attitude to staff, physicians and peers
  • Serves as a supportive change agent for Care Management model processes
  • Acts as a role model for Care Management department staff
  • Encourages team member engagement
  • Maintains open communication with staff
  • Assists with staff performance reviews/evaluations
  • Provides support and oversight with progressive corrective action plans
  • Balances schedules to meet department needs prior to posting
  • Communicates changes to expected schedule; minimizes unscheduled absences
  • Plans job responsibilities for maximum utilization of staff


Qualifications

:

Minimum qualifications:

  • Registered Nurse: BSN or ADN related Bachelor’s degree or
  • Social Worker: BSW
  • 2 years of experience in direct Care Management with increasing level of responsibility

Skills required:

  • Knowledge medical necessity for appropriate patient status and level of care
  • Knowledge of IMM, MOON, Advanced Directives, Multidisciplinary Rounds, Social Services, Value Based Programs, and other Care Management compliance guidelines
  • Ability to lead others effectively while maintaining appropriate compliance
  • Knowledge of Centers for Medicare & Medicaid Services (CMS) Conditions of Participation for discharge planning
  • Ability to manage multiple tasks and prioritize level of importance
  • Critical thinking skills
  • Customer service skills
  • Ability to implement process improvement processes
  • Experience with PC applications and development of reports
  • Ability to negotiate optimal resolution of issues
  • Ability to manage multiple tasks and prioritize level of importance

Preferred qualifications:

  • Registered Nurse: BSN
  • Social Worker: MSW
  • 3 years of experience in direct Care Management with increasing level of responsibility

Skills preferred:

  • Ability to engage in complex clinical decision making
  • Strong professional verbal and electronic communication
  • Ability to analyze, evaluate, and monitor data
  • Ability to foster positive Interdisciplinary Team relations and encourage employee engagement
  • Ability to work independently and/or as part of a team
