Advocate Lutheran General Hospital is seeking a full time Advanced Practice Registered Nurse to join their Inpatient Palliative Team

- Initial inpatient consults
- Advanced care planning, goals of care conversations and complex system management
- Provide Palliative education for medical staff, nursing and hospital staff as well as community
- Experience is Palliative Medicine preferred
- M-F days
- No weekends or on call but could be a future possibility

Position Requirements:
Masters of Nursing Degree as APRN~CNP
2 years RN experience preferred
1 year APRN~CNP experience in the relevant setting preferred
Well~developed interpersonal and communication skills.
Proficiency with an Electronic Medical Record
National certification as a CNP from an accredited body
Current Illinois APRN License
Current Illinois Registered Nurse license.
Current Illinois Controlled Substance License within 6 months of hire
Eligible for DEA registration Possession of a current DEA Registration within six months of hire and may vary based on practice site
Current BLS Certification
Current PALS/ACLS certification may vary based on practice site
Travel may be required
Ability to work varying hours and shifts to meet the needs of Advocate Health Care. This may include on call coverage ~Ability to work under stressful conditions and in difficult situations.
Able to respond quickly to patient needs and work at a fast pace ~Routinely works with potentially infectious specimens and patients.
Work environment includes probability of exposure to adverse, hazardous or unpleasant conditions while caring for the sick
This position can recognize needs and behaviors of a variety of patient age groups i.e., understand child growth and development, normal patterns of adolescent behavior and looks for signs of normal aging
Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case~by~case basis.
Purpose:
The APRN~Certified Nurse Practitioner (CNP) reflects the mission, vision, and values of Advocate Health Care and adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and other regulatory and accreditation standards. The APRN~CNP works independently and collaboratively as part of the interdisciplinary team to provide health promotion, disease prevention, health education and counseling as well as the diagnosis and management of acute and chronic diseases through high~quality, cost~effective delivery of care focused on population health. This includes the assessment, diagnosis, and management of patient problems by prescribing pharmacologic and non~pharmacologic treatments. The APRN~CNP is licensed, board certified, credentialed and/or privileged as an independent practitioner to practice at the full extent of their licensure and within their defined scope of practice. The primary goal of the APRN~CNP is to function in a clinical role to provide comprehensive population based health care services and may serve as a primary care provider and/or consultant for individuals, families and communities in a variety of settings.
