Position Requirements:
BA Degree
Demonstrated ability to work constructively with clients, treatment resources and the community. At least two years of relevant experience

Schedule: to be determined shifts may include evenings and weekends.
If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case~by~case basis.
Purpose:
To provide community based crisis intervention, case management and supportive services to patients of the Crisis Triage Service with emphasis on entitlements advocacy, resource development, housing, vocational training and employment supports. To participate in and support quality assurance and utilization review activities.
Accountabilities:
Case Management and Supportive Services Provides direct and indirect services to patients with emphasis on resource development, entitlements advocacy, outreach, and coordination of care.
Organizational and Training Activities Actively participates in organizational performance improvement, utilization review and incident debriefing activities in ways that make a positive contribution to organizational operations.
Recordkeeping Completes various clinical and administrative forms completes Certificate or Petition for those patients who require involuntary inpatient hospitalization. Assure accurate data collection for scheduling, billing, the management information system, and other purposes in accord with organizational policies and procedures.
Clinical Supervision Provides supervision to trainees of the treatment process and other professional duties. Directs trainees on approaches to patient needs or suggests alternative approaches for resolution of problems. Assists in training new staff.
