Aegis Health Care is currently seeking a dynamic Licensed Clinical Social Worker (LCSW) that has a passion for providing outstanding services to the elderly in Senior Living Communities and in-home health settings.
Aegis Health Care is currently seeking a Licensed Clinical Social Worker (LCSW) to join a home health, hospice, and geriatric medical team. The social worker provides psychosocial support to patients and families in patients home, care centers, group homes, and facilities. Must be knowledgeable with community resources as well as access to resources. Will be expected to assess, create a treatment plan, collaborate with the interdisciplinary team, and treat patients mental health needs. Will onboard and follow a caseload of hospice patients through end of life. Will assess and provide short term support for home health referrals. This is a self-directed role and demands a motivated and competent social worker. Able to work as a team however largely independent with the clinical social work needs.

Job Responsibilities:
- Develops and maintains therapeutic relationships
- Provides comprehensive psychosocial assessments and able to build care plans
- Provides supportive counseling- modalities helpful include CBT, ACT, MI
- Mobilizes community resources
- Provides effective patient/family/caregiver teaching
- Supports the aging/death/dying process
- Collaborates in providing patient care
- Creates timely and accurate documentation with strong clinical voice and documentation
- Assures patient safety- familiar and comfortable with crisis intervention and management- DTO and DTS assessments as needed
- Utilizes resources effectively and efficiently
- Adheres to hospice and palliative standards and works within the Code of Ethics
- Adheres to high standards of personal and professional conduct
- This position requires face-to-face visits with patients and the charting done via your location or in the field
- Observes, records, and reports changes in patients' condition and response to treatment to the team
- Attention to detail and able to problem solve and work independently
- Ability to work within an electronic medical record system (EMR)
- Ability to work effectively both independently and collaboratively as required
- Demonstrates flexibility with job duties and assignments as well as willingness to be a part of the team.
Skills & Qualifications:
- Current Licensed Clinical Social Worker (LCSW) certificate for clinical practice in the state of AZ
- Home Health / Hospice experience preferred
- Compassionate and caring attitude
- Keen attention to detail
- Service oriented mindset
- Valid Driver's license, clean driving record and reliable automobile
- Proof of current automobile insurance
- Criminal background check clearance/fingerprint clearance.
- Negative TB test
Salary : $50,000 - $90,000
