What the position looks like:
As a Home Health Nurse you will perform home visits to assess the patient’s health problems and needs as well as develop and implement a nursing care plan. You will be responsible for delivering quality medical care in compliance with industry/company regulations, policies and procedures. You will also be responsible for active case management of your case load.

What your responsibilities will be:

• Maintaining accurate, detailed reports and records using computerized documentation
• Administering medications to patients and monitoring patients for reactions or side effects
• Modifying patient treatment plans as indicated by patients' responses and conditions
• Monitoring all aspects of patient care, including diet and physical activity
• Instructing patients and families on topics such as health education or disease prevention
• Be able to work in a fast paced environment

What you will need:

EDUCATION
The minimum level of education desired for candidates in this position is an Associate's degree. Advanced certification or willingness to earn advanced certification preferred.
